If you prefer to submit a paper form for each child, English and Spanish forms can be obtained and/or returned to any HISD campus or by visiting the HISD Child Nutrition Services offices at 1010 7th Street, Huntsville, TX 77320. For questions regarding HISD school meals, please call Child Nutrition Services at 936-435-6920.
